Ein Jahr nach den Geschehnissen des ersten Teils kehrt Zatoichi (Shintaro Katsu) in das Dörfchen Iioka zurück, um dort im Tempel für den von ihm getöteten Ronin Hirate Miki zu beten. Doch dabei sind ihm die Häscher eines ranghohen Lords auf den Fersen, welchen Zatoichi zuvor massiert hatte und dabei seine geistige Behinderung entdeckte. Um diese peinliche Enthüllung zu verdecken heuern die Samurai den Yakuza-Boss Kanbei (Sonosuke Sawamura) an, um Zatoichi für immer zum Schweigen zu bringen. Um es noch schlimmer zu machen, befindet sich zu selben Zeit auch Zatoichis eigener Bruder Yoshiro (Tomisaburo Wakayama) im Dorf und trachtet Ichi nach dem Leben. Im Streit um eine Frau hatte Zatoichi den eigenen Bruder einst zum Krüppel geschlagen.
